Dr. David Mendelson, D.O.

Our Staff

Dr. David Mendelson, staff medical director, is board-certified in otolaryngology with oro-facial plastic surgery, and has been in private practice for over 20 years. Dr. Robert Miller, a former medical school instructor, is a board-certified otolaryngologist specializing in nasal and sinus disease, balance disorders and treatment of skin cancers of the head and neck. Ms. Linda MacConnell, physician assistant, has 21 years of Ear, Nose and Throat experience. Ms. Jill Juvan-Smith, audiologist, brings 16 years of audiology experience. Dr. Sadef Fateh, audiologist, recently completed an externship in Vestibular and Balance Disorders, Newborn Hearing Screening, and Amplification. Together, with our team of medical assistants and professional staff, ENT Specialists of Arizona is here to provide high quality health care for you and your loved ones. [Meet our staff]

Insurance

Our office accepts payments from most insurance companies. We do request that you bring your insurance card, valid drivers license or picture ID, and a billing address so that we are able to collect important medical information. We accept most credit cards for easy payment.

In some cases your insurance plan will require a Referral Authorization to allow you to see our physician. It is important to follow the referral process. If the referral is not followed, you may be responsible for some or all of the additional costs. We appreciate and value referrals from our patients and friends.
